چکیده

The malposition of a self-expandable metal stent (SEMS) placed fully inside the bile duct may be disastrous, and adjusting the position of a malpositioned stent can be difficult or impossible. Fully-covered SEMSs were designed mainly to treat benign biliary strictures and to be removable [1]. The removal process can however be difficult if the stent is placed fully inside the bile duct. Grasping the end of stent inside the bile duct directly with biopsy forceps may allow metal stents to be easily adjusted or removed. However, the front tip of a biopsy forceps is straight and hard, so access to the bile duct with biopsy forceps is difficult, especially via a native papilla without a sphincterotomy. Here, we present a simple method that facilitates the entry of biopsy forceps into thebile duct. First, a guidewire is deployed into the bile duct. Second, a serrated-edge biopsy forceps is used to grip the guide wire outside the papilla (●" Fig.1a,b), and is then slid into bile duct while still gripping the guide wire (●" Fig.1c). Finally, the forceps is used to grip the metal stent under radiographic guidance and to adjust or remove the stent as required (●" Fig.2). This guide wire-assisted technique for access to the bile duct with biopsy forceps is an extremely effective and simple technique that makes the adjustment and removal of metal stents placed within the bile duct more convenient and safe. Guide-Wire Assisted Endobiliary Forceps Biopsy Sampling

Biopsy procedures for sampling of the proximal bile duct during endoscopic retrograde cholangiopancreatography (ERCP) can be technically difficult. Insertion of biopsy forceps into the bile duct is not an easy task and can cause some adverse events. In addition, controlling and positioning the forceps can be difficult during targeted sampling of a designated area. If we could easily insert biop...

The Wire-Grasping Method as a New Technique for Forceps Biopsy of Biliary Strictures: A Prospective Randomized Controlled Study of Effectiveness

BACKGROUND/AIMS Transpapillary forceps biopsy is an effective diagnostic technique in patients with biliary stricture. This prospective study aimed to determine the usefulness of the wire-grasping method as a new technique for forceps biopsy. METHODS Consecutive patients with biliary stricture or irregularities of the bile duct wall were randomly allocated to either the direct or wire-graspin...

Endoscopic removal of an impacted bile duct self-expanding metal stent (SEMS) using the SEMS-in-SEMS technique.

In contrast to covered self-expanding metal stents (c-SEMS), retrieval of uncovered SEMS (u-SEMS) from the bile duct may be quite difficult or impossible. We describe the endoscopic removal of an impacted u-SEMS using the SEMS-in-SEMS technique. A 62-year-old man presented with acute pancreatitis, jaundice, and weight loss. Computed tomography (CT) revealed a “malignant-appearing” mass in the h...

Evaluation of guide wire cannulation in reduced risk of post - ERCP pancreatitis and facilitated bile duct cannulation

Background: Pancreatitis is most common complication of post-ERCP and needs to admission at least for one day. The purpose of this study was to assess the efficacy of guide wire for better common bile duct (CBD) cannulation for reducing post-ERCP pancreatitis. Methods: From April 2010 through March 2011, the patients who needed ERCP and referred to Shahid Beheshti and Rouhani Teaching Hospita...
